MRI Appointment Communication Consent

The Imaging Center that scheduled your MRI appointment uses our services to conduct a screening and perform an MRI Safety risk assessment to ensure your procedure can be conducted safely. 

 

 

By providing your contact information and giving verbal consent to the Imaging Center, you consent to receive text messages and emails from us regarding MRI screening and appointment details. These communications are intended to ensure your safety and to provide important updates about your MRI procedure.

 

If at any point you wish to opt out of receiving these communications, please inform the Imaging Center where your MRI appointment was scheduled, and they will update your preferences accordingly.

    By accessing or using the Service, you acknowledge that you have read, understood, and agree to these TOS. This TOS applies to all patients who use the Service.

    a. Eligibility: You must be at least 18 years old to use the Service. If you are under 18, your parent or legal guardian must create and manage the account and use the Service on your behalf. By using the Service, the parent or legal guardian agrees to these Terms of Service and assumes responsibility for the minor’s use of the Service.

You are responsible for maintaining the confidentiality of your account information, including but not limited to your username, password, and any other credentials associated with your account. Failure to protect this information may result in unauthorized access to your account, for which you assume all associated risks.

    Any disputes arising out of or related to these TOS or the Service shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association (AAA). The arbitration shall take place in Maryland, and the decision of the arbitrator shall be final and binding on both parties. Each party shall bear its own costs and fees related to the arbitration.

    These TOS and any disputes arising out of or related to these TOS or the Service will be governed by the laws of Maryland, United States, without regard to its conflict of laws principles.
